conduct an assessment of the health status of Barangay Daniel C. Mantos of the Municipality of Mahayag for the formulation of a comprehensive health plan that aims to mobilize the people, the local government and other non-government units and sectors for the improvement, strengthening and sustenance of the community people’s health thru empowerment and self-sustaining programs wherein the community people will take on active participation and management of their own health.

Because waterborne diseases are considered burdensome to human health, improving access to safe drinking water is considered to be one of the most important public health goals. Based from data gathered, the problem of unsafe drinking water in Barangay DCM may be from an unsafe source, poor protection of the water source or lack of water purification methods. The first important step is an assessment of the potability of all identified water sources through bacteriological testing.Next, efforts should be geared towards establishing a system for maintaining and ensuring the potability of the water supply, because a clean and safe source can still be made unsafe if no actions for its protection and maintenance shall be done. It is important that safe water sources are protected and maintained while unsafe water sources must be given appropriate treatment or closed permanently. It is also important that the people are made aware of the importance of safe and potable water so that in turn their behaviour and practices shall be modified. The “Water Mo? P-PuSH mo!” campaign shall be launched with the aim of providing information regarding the proper Protection, Purification, Sanitation and Handling of drinking water.

Waste disposal takes a long process, from households to dump trucks and down to dumping sites. The budget allotted for garbage collection will put in waste so as garbage.  The budget generated for garbage collection can be productively use in other significant infrastructures such as schools and hospitals; and other health related outreach such medical missions and giving out free medicines to less privileged community. Moreover, Barangay DCM only practice burning and compost pit as a main method of waste disposal which entails detrimental effects on health and environment. Therefore, there is a need for an efficient Solid Waste Management Garbage Disposal and Segregation Plan. After gathering data on exiting law ordinances, proper regulation and implementation of laws must be executed by barangay officials in authority. Aside from this, identification of existing practices regarding on waste disposal and enhancement of the community on proper waste disposal including advantages and disadvantages of each waste disposal method through general assembly pursue. Lastly, re-initiation of garbage collector system with the aid of barangay officials and local government units as well as non-government units must be conducted.

Malnutrition is one of the leading contributory factors to child mortality; it increases a child's susceptibility to disease and it also contributes to delayed growth and development. To address the poor nutritional status of the children under 5 in Barangay DCM, it is first important to raise awareness about the existence of the problem by conveying to the people, especially the parents the information regarding the children's nutritional status. It is also vital to include the barangay health workers in the process by establishing a team that will be in charge of the implementation of nutritional programs for children. The launching of the Ka-UN Kalusugan Ug Nutrisyon campaign will help raise awareness and provide knowledge on the importance of good nutrition through information dissemination for the people in the community.

Hypertension is the top leading cause of morbidity in Barangay DCM. According to our findings, 24.2% of households in Barangay DCM have at least one member with hypertension. What's more alarming is, majority of these individuals are non-compliant with their prescribed maintenance medication, which predisposes them to developing deadly complications associated with elevated blood pressure. The aim of our plan is to diagnose hypertension among individuals with elevated blood pressure and improve their compliance to anti-hypertensive medications.

Rabies is an acute fatal infection that affects both humans and animals, with dogs as the main source of human deaths. Although highly preventable, mortality is still high worldwide. Although there are no reported cases of rabies in Barangay DCM, it is still a perceived problem due to the high risk of the residents for dog bites and probable cases of rabies. A core group shall be established for the monitoring of dog bite cases and responsible pet ownership. Seminars for responsible pet ownership shall be conducted together with mass dog vaccination and registration of pet owners. Strict implementation of an ordinance regarding responsible pet ownership will also help lower the risk for dog bites in the community.
